Pre Exercise Questionnaire

Thanks so much for taking the time to complete this form.  I appreciate it can be overwhelming, however as I’m a Level 3 Registered Fitness Professional with Fitness Australia, it’s a legal requirement prior to undertaking exercise instruction from me.  All information is kept confidential as per the Privacy and Personal Information Act.

On the other hand, it allows me to better understand what is going on for you and your body which means I can incorporate more specific exercises tailored to your needs during the sessions, supporting you to improve as a rider.  If you have any specific concerns, please don’t hesitate to contact me immediately.
